- Must enroll in the courses as mention in the list of CHED Priority Program.

- Certified True Copy of Grades: Usually includes Grade 11 and the first semester of Grade 12 grades for graduating students.
C. Income Documents
- ITR (Income Tax Return) – of parents or guardians, if available.
- Alternative Income Proofs:
- Certificate of Tax Exemption: From the Bureau of Internal Revenue (BIR) for those exempted from filing taxes.
- Certificate of Indigence: From the barangay or the Department of Social Welfare and Development(DSWD) if the family belongs to a low-income bracket.
- Latest Contract or Proof of Income: for children of Overseas Filipino Workers (OFWs) and seafarers.
D. Other Documents for Special Groups (if Applicable)
- PWD Identification Card or Certificate if applying as a person with a disability (PWD).
- Solo Parent ID or Certification for solo parent application or their dependents.
- Other Government certification if applicable e.g:
- indigenous people,
- underprivileged sector.
E. CHED Scholarship Application Form
- Application Form: This can be obtained from the CHED Regional Office or the Official CHED Website. The Form needs to be Completed and Signed.
CHED Scholarship Benefits
The CHED Scholarship program benefits are typically divided between two main types of scholarship as follows:

- Scholars will receive a total of ₱120,000/Yearly.
- Tuition and other school fees – ₱40,000.
- Stipend – ₱70,000.
- Book and Connectivity allowance of – ₱10,000.
Semester Benefits:
- Semester Benefits will receive a total of – ₱60,000:
- Tuition & Other School Fees – ₱20,000;
- Stipend – ₱35,000;
- Book and Connectivity allowance – ₱5,000.
Half PESFA – Yearly Benefits
- Will receive a total of ₱60,000 Given as follows:
- Tuition & other school fees – ₱20,000;
- Stipend – ₱35,000;
- Book & Connectivity allowance – ₱5,000.
Half PESFA – Semester Benefits
- will receive a total of ₱30,000, Given as follows:
- Tuition and other school fees – ₱20,000;
- Stipend – ₱17,500;
- Book & Connectivity allowance – ₱2,500.

Full SSP – Yearly Benefits
- will receive a total of ₱80,000, Given as follows:
- Tuition and other school fees – FREE;
- Stipend – ₱70,000;
- Book & Connectivity allowance – ₱10,000.
Full SSP – Semester Benefits
- will receive a total of ₱40,000, Given as follows:
- Tuition & other school fees – FREE;
- Stipend – ₱35,000;
- Book & Connectivity allowance – ₱5,000.
Half SSP – Yearly Benefits
- Will Get a total of ₱40,000, Given as follows:
- Tuition & other school fees – FREE;
- Stipend – ₱35,000;
